To remove the Company name from the account, please submit the following documents:

a) Independent Distributor Application Form, updated, signed and dated (form available in the BackOffice under the ‘Documents’ section).
b) Copy of W9 Form (updated, signed and dated).
c) Copy of individual’s Tax ID Certificate, issued by the country’s official tax office (IRS for USA).
d) Signed letter from the distributor requesting to remove the company name from the account.

Please, keep in mind that ID numbers must be referenced in all correspondence with the company.
