Adding a Company Name (DBA)

The following is needed for the Company name:

a. DBA Form- (form available in the BackOffice under the ‘Resources’ section)

b. Independent Distributor Application Form, updated, signed, and dated (form available in the BackOffice under the ‘Resources’ section).

c. Copy of W9 Form (updated, signed, and dated).

d. Copy of Certificate of Incorporation, Partnership Agreement, or Trust Documents

e. Copy of Company’s Tax Certificate, issued by the country’s official tax office (IRS for USA, Secretaría de Hacienda Mexico)

f. We may also request Proof or Certificate of the legally entitled person to do business on behalf of the company, in case that the applicant’s name does not appear on the Tax Certificate. This person should not have any interest in any existing OrGano Gold account.

Please, keep in mind that ID numbers must be referenced in all correspondence with the company.
